Hi,
I created a form for my team to complete when screening donors. How can I ensure that we receive a PDF or downloadable version of the completed form with each email submission?

To receive a PDF or downloadable version of the completed form with each email submission, you can enable the PDF attachment option in your email notifications. Here's how to do it:
- Go to Settings in your Form Builder.
- Select Emails from the left menu and click the Pencil icon to edit your notification or autoresponder email.
- Go to the Advanced tab and enable the PDF attachment option.
- Save your changes.
This will attach the submission as a PDF to each email you receive. Alternatively, you can add a PDF download link in the email content by inserting the {pdf-link} tag.

Hi Donoe_Dept,
Let me show you how to attach a PDF to your Notification email:
- In Form Builder, click on Settings on the orange navigation bar at the top of the page and select Emails from the left side window.
- Next, click on the Pencil icon of the Notification email.
- From the Advanced tab, toggle Attach PDF and select the PDF that you want to attach to the email.
- Lastly, click on Save at the bottom of the page.

Is it possible to embed a PDF consent form that requires a signature within this form? For example, our team uses this form to screen donors, and at the end, we’d like the donor to sign a consent form. Can we either attach the PDF or include the consent section at the end of the form for signature? Any suggestions on how to include the consent form.

Hi Donor_Dept,
Yes, you can add consent and signature sections to the end of your form. Here's how to do it:
- In Form Builder, scroll down to the end of your form and click on Add Element on the top left side of the page.
- Then, under the Basic tab, drag and drop the Paragraph element to your form.
- Next, drag and drop the Signature element under the Paragraph element you've just added.
- Next, double click on Click to edit this text..., paste your consent text inside the text editor, and click on the Checkmark icon on the right side of the field.
Check out the text editing options inside the Paragraph element's text editor and let us know if you have any questions. Keep in mind that adding the Signature field enables Sign Automation for your form and doesn't allow submissions to be edited. This is necessary for the signature to be lawfully binding.
