Before I sign up for a plan I would like to know if Jotform can create an assessment with 12 dimensions, 6 items per dimension with Likert 1-5 scoring in which 2 of the 6 items are reverse scored. For example:
Dimension 2: Emotional Dysregulation
Reverse score item 2,5.
- I feel emotionally numb or disconnected at times.
- I usually feel stable and emotionally grounded.
- Small criticisms can feel devastating or overwhelming.
- I am often told I’m “too sensitive” or “too intense.”
- I am able to calm down quickly after getting upset.
- My emotions change rapidly and sometimes feel out of control.
a. Can the system handle reverse scoring?
b. If all dimensions were combined into a 72-item questionnaire with items presented randomly, could Jotform gather the appropriate items into the correct dimension? For example labeled 2-1, 2-2, 2-3 etc for Dimension 2?
c. What level of feedback is available based on scores? Raw scores? Is a text feedback possible based on scores of e.g. low 0-10, moderate 11-20 and high 21-30?

Hi Drdrjanet,
Here’s how Jotform can support your requirements:
- Reverse Scoring: Jotform does not have an automatic reverse scoring feature. However, you can manually assign calculation values to Likert scale items and use calculation fields or conditional logic to adjust scores for reverse-scored items.
- Randomizing Items and Grouping by Dimension: You can combine all 72 items into a single form and randomize their order. By labeling items according to their dimension (e.g., 2-1, 2-2 for Dimension 2), you can later group and analyze responses by dimension using Jotform Tables’ grouping features.
- Feedback Based on Scores: Jotform allows calculation of raw scores through calculation widgets or conditions. You can set up conditional logic to display customized text feedback based on score ranges (e.g., low, moderate, high) for overall or dimension-specific scores.
Jotform’s drag-and-drop interface and powerful calculation and condition tools make it possible to build complex assessments without coding.
Give it a try and let us know how it goes.
