Hi GOLFSelect_golfselect,
Thanks for reaching out to Jotform Support. The error message "Value must be an option index or uuid" indicates that one of the fields in your ClickUp task creation step is expecting a specific type of value, such as an option index or a universally unique identifier (UUID), but it received something else.
You can check data for failed submissions to ensure all data sent from Jotform can be accepted by fields in your ClikUp account. Ensure that all required fields on ClickUp are filled in and that the format is correct. If you have fields with predefined values (like single choice or dropdown) ensure that all values received by your form are the same.
If the issue continues, let us know how about your ClickUp fields, so we can check them further. It would be helpful if you could share a screenshot of them, so we get better understanding of what's going on.
Give it a try and let us know how it goes.

Hi GOLFSelect_golfselect,
Can you check and ensure that the Package Selected field in Click Up is also a single choice field, or equivalent, and that all choices also matches the options in your form as seen below?
Let us know how it goes.

Hi Mary,
Yes, I had checked that. However I deleted and recreated the field and it is now working. My only issue now is that I have a new zealand mobile number that is not being recognised. Do you know what it is supposed to be to integrate into clickup? i have the input mask as (+64) ## ### ####
thanks

Hi GOLFSelect_golfselect,
The New Zealand mobile number is not being recognized. The format you shared generally works for landlines, but it might not always align with mobile number validation, especially for integrations like ClickUp that expect numbers without formatting. For better compatibility, I recommend using this format instead:
- +64######### (no spaces, parentheses, or special characters)
- For example +64211234567
You can try removing the input mask and using a Short Text field instead to ensure the number passes through cleanly to ClickUp?
Give it a try and let us know how it goes.

Janet BowmanReplied on August 1, 2025 at 11:05 PMHi Support,
Thanks for the previous message.
Let me explain what I am trying to achieve in more detail.
I have created a jotform to collect information from a number of companies represented as a dropdown list. The companies reside in Aust and NZ – these have different mobile number formats.
I have been having issues with the NZ mobile number format – as per the previous threads so I have taken the advice and created the NZ mobile as a short text.
I changed the jotform so that when the NZ company is selected from the dropdown – it shows the short text mobile field instead of the mobile number field. However when I complete the form for a New Zealand company it is still coming up with the error – Value is not a valid phone number. I turned the mobile number field for Australian mobiles to not mandatory – and changed the field name – I’m not clear from the error message as to what number is being referenced.
Any advice would be appreciated.
Kind Regards
Janet Bowman
