Regarding integration with Modmed. Currently, Jotform does not have a native integration with them. If you would like, you can explore alternative methods such as using third-party automation tools or APIs to connect Jotform with Modmed.

At this time, Jotform does not have an existing integration available. In
order for your practice to integrate with Jotform, they would first need to
develop an integration and go through the vendor approval process. Once
approved, they would then be able to integrate with your practice via the
API.

While the feature you’re looking for isn’t currently available at Jotform right now, we've gone ahead and escalated your request to our Developers.
