Now, coming back to your question, I checked and tested your form to see if I could replicate the issue, but everything was working properly. Check out the screenshot below to see my results:
The issue in your screenshot is more about the images not fully loading. This is usually related to the internet connection you have or the browser's cache. I suggest connecting to a different, stable network or clearing the browser cache, as it helps with temporary issues. You can also clear the cache of your form to help on this. Although Clearing Your Form Caches isn't done automatically, it's easy to do it manually. Let me show you how:
1. On your My Workspace page, click on your Avatar/Profile Image icon.
2. In the dropdown menu, click on Settings.
3. In the Settings menu, click on Clear Cache. And that's it, you're done.
