I want to have a solution for paper forms that we currently have to get filled in our business. The form is to be completed by two people (customer and us) and we both need to sign the form. It is a clinical form so there are fields that a tick box would be used, other fields are to add txt

You can create a form on iPad that requires signatures from both the customer and your team using Jotform's e-signature features. In the Form Builder, add the Signature element twice—one for each signer. You can also include tick boxes and text fields as needed for your clinical form.
For a more streamlined process, consider using Jotform Sign, which allows you to send documents for signature and track the signing workflow easily. This tool is compliant with electronic signature standards and supports multiple signers.
Here’s how to add signatures:
- Open your form in Form Builder.
- Select Add Form Element and choose Signature to add it to your form.
- Repeat to add a second Signature element for the other signer.
This setup will allow both parties to sign directly on the iPad. You can also add tick boxes and text fields to complete your clinical form requirements.

Hi Marcelo,
To replace your current paper forms, we recommend using a regular online form with fields for text input, checkboxes, and signature fields—one for the customer and one for the employee. The sign document is currently not supported in kiosk mode.
If both the customer and employee will be signing the form at the same time in the clinic, you can enable Kiosk Mode to streamline the process. This mode automatically reloads the form after each submission, making it ready for the next client without manual resets.
On the Jotform Mobile App, you can use Offline Mode to save your form to your device for filling out, or you can use Kiosk Mode, even without an internet connection. All submission data will be saved to your device and sent to your Inbox once you're connected to the internet. Let me show you how to set it up:
1. On your My Forms page, tap on the form that you want to access in Offline Mode.
2. On the bottom-right side of the menu that opens, toggle Offline Mode to the On position.
You can't use certain widgets and elements in Offline Mode because they require an internet connection to function properly. Let me show you how to use Kiosk Mode when you don't have an internet connection:
1. On your My Form page, tap on the form that you want to open in Kiosk Mode.
2. About halfway down in the menu that comes up, tap on Kiosk Mode.
3. Adjust the Kiosk Settings as you like and then tap on Launch Kiosk at the bottom. And that's it.
