Hi ariabiospa,
Thanks for reaching out to Jotform Support. I cloned your form and tried to share it as a template to replicate the issue, but everything seems to be working correctly. Check out my screencast below:
I also tried to assign it to another email address, and it also worked correctly. When you share a form as a template, the recipient should be able to open the link of the form to their account. Can you share with us the template share link you used so we can try to access it on our end? I also cleared your form cache as a precaution.
After we hear back from you, we’ll have a better idea of what’s going on and how to help.

Hello,
this is the link that was emailed to my lerevespala@gmail.com account:
It still doesn't work. I'm not sure why this keeps happening.
Please help me, if possible can you send the form to the requested email? I also tried to send the Le Reve sales waiver and wasn't successful..

Hi ariabiospa,
I’ll need a bit of time to look into this. I’ll get back to you as soon as I can.

Hi ariabiospa,
When I tried to access the URL you shared, I got the same issue. When you share a form as a template, it goes through a review process before it is published and accessible to others. Until it is approved and published, the template remains private and will show an error if accessed by others. Unfortunately, we could not send the form to the email address.
If you want to let another user have the same format of your form, you can ask them to clone your form instead. Cloning an Existing Form From a URL is easy. Let me walk you through it:
1. On your My Workspace page, click on Create on the top-left side of the page.
2. Select Form from the assets, and then click on Import Form.
3. In the next window, click on From a Web Page.
4. Paste the form link into the Enter URL field and click on Create Form.
Give it a try and let us know if you need any other help.
