I'd like to add a slider type question. As it is a rating scale, I'd like to have explanation. For example: Here is the question,

Thank you for reaching out to Jotform Support. I understand that you're looking to add a slider-type question with a rating scale and include explanations or descriptions for the scale values in your form. I’ll need a bit of time to look into this. I’ll get back to you as soon as I can.

Currently, a slider with text labels for the low and high rating that registers numerical input isn't supported. The feature to do that isn't currently available, but I can see how useful that could be.
I've gone ahead and submitted a feature request for that, and we'll be sure to follow up with you here if that functionality is added in a future update. In the meantime, I can walk you through a workaround.
Using our Scale Rating element, you can describe the Lowest and Highest values. Let's walk through setting this up together, below.
- In Form Builder, under the Build tab, click the. +Add Element button.
- Click and drag the Scale Rating element to the form.
- Change the Field label to "When it comes to transforming how our team operates, our leaders are..."
- Click the Gear Icon.
- Select the Options tab.
- Under Lowest Rating Text, enter "Mostly looking for others to change (value 1)"
- Under Highest Rating Text, enter "Eager to be challenged and change themselves (value 10)".
