I am looking for a simple program that allows me to fill out an Insurance 1500 form on my computer and then print the info on to a pre printed red-ink Form 1500 without involving Adobe Acrobat.
Is that something you have?
Thanks

Hi James Anderson,
Thanks for reaching out to Jotform Support. I understand you want to create an Insurance 1500 form. This is possible through Smart PDF Form. It allows you to import a PDF file and will generate form fields that users will fill out.
Let's walk through this together in creating a Smart PDF Form:
- From My Workspace, click the orange + Create button.
- Select Smart PDF Form.
- Import the PDF file by clicking the Upload Document button or dragging it.
Form fields will be automatically created and mapped to the original PDF file fields. We also have this helpful guide on How to Link Your Online Form Fields with PDF Through Smart PDF Forms.
However, the requirement you need without showing red fort color is unsupported. Jotform isn't in control of the original PDF file that's imported. You'll need to edit it on another platform before importing it to the Smart PDF Form.
If that’s not exactly what you’re looking for, can you explain more information so I can help you better?
Let us know if you've got other questions.

Hi James,
Great question yes, there are a few tools out there that let you fill out a CMS-1500 (HCFA) form digitally without needing Adobe Acrobat. While Jotform does offer customizable PDF templates and online form builders, printing directly onto pre-printed red-ink Form 1500s can be tricky, since alignment is so precise.
You might want to try creating a custom form with exact field alignment using Jotform’s PDF Editor. Then you can use your browser’s print settings to match it to your paper form. Some medical billing services or specialized insurance platforms also offer built-in CMS-1500 tools.
If you're working with multiple patient forms, claims, or insurance data, this has helpful resources for streamlining paperwork for families and small practices worth a look if you're dealing with multiple submissions regularly.
Let me know if you'd like help customizing a form layout for exact field positions.
