Now, coming back to your question, are you referring to the form named Lista di prelievo? When I cloned and checked it, I saw that you’ve already added an Input Table and set up the columns. With the Input Table, your users can enter text in each column. Check out my screencast below to see my results:

But if you don’t want to use the Input Table element, you can try adding a Configurable List to your form. It also lets users add new rows directly within the widget. The Configurable List widget is easy to use. Let me show you how:

1. In Form Builder, click on the Add Element menu on the left side of the page.
2. Under the Widgets tab, drag and drop the Configurable List widget into your form or just click on it to add.
3. In the menu on the right, customize the Fields Configuration section according to the syntax below.

After that, to make the rows look like columns, set the Label Align to Top. Here's how to do it:

1. In Form Builder, click on the Configurable List widget, then click on the Gear icon to open the Question Properties.
2. Under the Label Align, click on the Downward Arrow icon.
3. Next, in the dropdown menu, select Top.
