Can I set up a table with the specific headings?

Can I set up a table with the following headings:

Submission Date; Product ordered; Qty; Subtotal; Tax; USD Total; First Name; Last Name; Email; contact #

It is possible to create columns with your following headings except for the tax,subtotal.

This will also depend on the form you're creating.

Your form should have the fields, name,email,phone number and product field to have such columns.
