Jotform integrating with Monday.com question

How do I get the Full Name, Supervisor Name and Address lines to populate out individually? In Jotform, it captures each as one main field (and then individual fields) but when I am matching them in the integration, I need it separately (First Name) and (Last Name). Same with Address (Street Address 1, City, State, Zip Code. I see that it has a grayed-out selection in my integration but doesn't allow me to select it.
