Hi! We are using you for patient inquiry forms and I know you have that you are HIPAA compliant and looking to have you sign a BAA with our team. How can we go about doing that?

To get a BAA signed for your HIPAA-compliant account, you first need to enable HIPAA compliance features, which are available for Gold plan subscribers. Once enabled, you can submit your details to receive the signed BAA via email. You might want to consider upgrading your plan if needed, and you can find more information about this by using the Upgrade Gold Plan button below. You can also check other plans by the Check Pricing Details button.

We’ve checked your account and found that you are currently on the Bronze plan. Currently, HIPAA compliance features are available starting with the Gold plan, so you may consider upgrading to Gold to access them. Let me show you how to upgrade your account:
1. Log in to your Jotform account, go to the Pricing page, and click on Get Started under Gold subscription.
2. In the Choose Your Payment Schedule section, select Yearly or Monthly.
3. Then, enter your billing details in the Billing details section.
4. In the Choose your Payment Method section, select Credit/Debit Card or PayPal.
5. If you choose Credit/Debit Card, you can enter your Credit/Debit Card information below the Credit/Debit Card option.
6. Next, in the top-right corner of the page, in the Summary section, tick the box next to I’ve read and agree to the Terms of Service, and click on Pay.
But, if you select PayPal as the payment, a PayPal Payment window will open, where you can log in and complete your payment after clicking on the Pay with PayPal button. Check out this screenshot below:
After the subscription is successfully paid, the features and limits for the Gold plan will be applied to your account right away, allowing you to enable HIPAA compliance and receive your BAA afterward.
