I fixed the other text box issue, but the form needs to have all fields and the response as well they are not synching properly

Thanks for reaching out to Jotform Support. When I cloned and tested your form, everything worked the way it was supposed to. The notification email contains the all the form fields set up in the email content. Take a look at the screencast below to see my results:
Ensure that all fields you expect to see in the responses are properly added and configured in the form builder. If the Notification Email isn't showing all the fields, removing it and adding it back usually fixes the issue. First, let me show you how to delete it:
- In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
- In the menu on the left side of the page, click on Emails.
- Hover over the Notification email, click on the Gear icon, and click on the Trash Can icon.
- In the window that opens, click on Delete and that's it.
- While in Emails, click on the Add Email button and select Notification Email.
- Once you've customized the new notification email, click the Save button.
