El código del widget QR dinámico no funciona

He creado este sencillo formulario para probar el widget, en donde se pretende a partir de un campo de valor aleatorio generar un QR , no sé si estoy utilizándolo mal ya que lo he enlazado con el nombre del campo, sin embargo suceden dos cosas:

1. No muestra ninguna imagen QR.

2. Hace desaparecer todos los componentes que he colocado por debajo del widget.

Alguna solución...

Now, coming back to your question, I tested your form and had the same issue, so I created a ticket and escalated this to our Developers. Although we don't know exactly when it'll be fixed, we’ll circle back to this thread when we have an update.
