Hi when i click "Electrics" at the risk assesment. The personal protective equipment that must be used should be the mandatory ones + Isolate electrics. But for some reason when i click Electrics it ticks all the boxes for "Hot work" too. Also any combination with electrics doesnt work properly.

    Hi Alex,

This is the only condition you need to add to your form to automatically select "Isolate electrics" when "Electrical" is selected as a Risk Assessment option.

    Here's how it works:

All other conditions that are currently set to select "Isolate electrics" should be removed or disabled. You can apply the same logic to create conditions for the other Risk Assessment options as needed.

Let me walk you through how to disable or delete a condition in your form:

    1. In Form Builder, click on the Settings tab in the orange navigation bar at the top.
    2. Select Conditions from the menu on the left side of the page.
    3. Beside the search box, click on the All Types dropdown and choose Update/Calculate Field.
    4. Enable the Select All checkbox.
    5. Click on Disable to disable the selected conditions.

I’ve made these changes in a cloned version of your form. Here is the link to the clone form. If needed, you can clone the form to your own account using the How to Clone an Existing Form From a URL guide.

Give it a try and let us know how it goes.

But now it doesnt work in combination with others. for example i want Hot work and electrics, The PPE should be isolate electrics, Barriers, Face shield, Fire extinguishers, Overalls, Area clear of waste, Gas chemist certificate, Isolate fire alarm, Breathing apparatus.

And also i dont understand the condition you make because you never choose "is equal to" electrics. But just said "Filled". How does the form now that electrics was chosen then.

    Hi Alex,

I checked out your form and saw you’re creating conditions for every possible scenario. Because of that, the conditions end up conflicting with each other, which makes some selections not work the way you want. You don’t really need that many conditions to get it working. You only need one. Let me show you how:

    1. Open your form in Form Builder, then click on Settings in the orange navigation bar at the top.

    2. Choose Conditions in menu on the left side of the page.

    3. Next, filter the conditions to only show the ones linked to the Risk Assessment field using the Form Fields filter.

    4. Check the box next to Select All, then click on either Disable or Delete.

    5. Now, click on Build in the orange navigation bar at the top, then click on Risk Assessment field to select it.

    6. Click on the Gear icon to open Multiple Choice Properties, then click on the Options tab.

    7. Toggle Calculation Values to On position, then add the options that need to be selected in the Personal Protective Equipment field when that option is chosen. For example, you can add the texts below to the Work at Height option's calculation values:

Safety harness, Radio communication, Protection from falling objects, Barriers, Area clear of waste, Item defect checked before use, No high winds, Ladder footed at all times

Just make sure they match the exact options in the Personal Protective Equipment field and separate them with a comma.

    8. Repeat the same steps to the other options.

    9. After adding all calculation values, click on the Add Element menu on the left side of the page.

    10. Under the Widgets tab, drag and drop a Form Calculation widget onto your form, right below the Risk Assessment field.

    11. Click on the default label and rename it to any descriptive label you want to use.

    12. Now, click on the Wand icon to open Widget Settings, then click on Add Field in the menu that opens to the right.

    13. Choose the Risk Assessment field from the list, then click on it to change its value to texts, and click on Save.

    14. Right -click on the widget, then choose Hide from the options.

Now that the fields are set up, create a condition that passes the widget’s value to the Personal Protective Equipment field once the Risk Assessment field is filled. Here's how to do it:

    1. While in Form Builder, right-click on the Form Calculation widget, then choose Conditional Logic from the options.

    2. In the window that opens, click on Add Condition, then choose Update/Calculate Field in the screen that opens next.

    3. Set the condition as you see below.

And that's it. Check out the screencast below to see how it works now on my cloned form:
