Hi,
Do you have a template or can you tell me how to create a jotform that has an image with a yes / no answer underneath it. I want the yes / no answer to propagate the same cell in a google sheet with either yes or no.

Hi Grow,
Thanks for reaching out to Jotform Support. Let me help you with that:
- Create a form and add an Image element to upload your picture.
- Add a Single Choice field below the image with options "Yes" and "No".
- Integrate your form with Google Sheets to have the responses, including the yes/no answer, populate the same cell.
This setup will allow you to collect yes/no answers linked to the image and send them directly to your Google Sheet.

Instead of typing a question I want to upload an image to a single choice field

Hi Grow,
In Jotform, you cannot upload an image directly as an option inside a Single Choice field. Instead, you can add an Image element above or near the Single Choice field to display the image, and then place your yes/no options below it.
This way, the image is shown clearly, and users can select yes or no as their answer.

Hello,
Unfortunately you can't add images to the single choice options. You can try giving some of the widgets a try. Some widgets I found were the Image Picker widget, Image Checkboxes, and Image Choices.
Hopefully one of those could work for you.
