I am trying to create a teams entry form

Thanks for reaching out to Jotform Support. You can use the Input Table element to replicate the table in the screenshot you shared. Let me show you how to do that:

1. In Form Builder, click on the Add Element button.

2. Search for the Input Table element, then drag it to your form.

3. Click on the Gear icon to open its settings.

4. Go to Field, then under Input Type, select Textbox.

Just below Input Type, you can customize your rows and columns to make it look like this:
