Hi LLC,
Thanks for reaching out to Jotform Support. I'm an AI assistant. I'll do my best to help you until someone from the support team is available.
In Jotform, you can achieve a lookup function to populate fields based on a selected value by using Tables Prefill. This feature allows you to connect your form to a data table where you have stored your account numbers along with addresses, phone numbers, and other details.
Here’s how to do it:
- Go to Publish in your form builder.
- Select Prefill on the left side.
- Choose Tables Prefill and select the source table containing your data.
- Map the columns from your table (like account number, address, phone) to your form fields.
- Save the settings and create a new prefill to link the data.
This way, when a user selects an account number, the related fields like address and phone number will automatically populate.
Give it a try and let us know how it goes.

Hi forms_forms4,
Thanks for reaching out to Jotform Support. We have the Spreadsheet to Form Widget that you can use. Since you already have the Excel File, you can now add it to your form. Let me show you how to use it:
1. In Form Builder, click on the Add Form Element menu on the left side of the page.
2. Click on the Widgets tab, search for the Spreadsheet to Form widget and add it to your form.
3. In the menu on the right side of the page, upload your data file. (You can simply drag & drop your Excel file to the Upload File area.)
4. Then, click on the Downward Arrow icon in the Access Code Column option and select the Account Number column.
5. You can also customize texts used in the widget. After you complete everything, click on the Update Widget button at the bottom right of the menu.
