Hi Richard,
Thanks for reaching out to Jotform Support. Let me help you with that:
To set up your multi-stage form workflow with conditional formatting and page visibility, you can use Jotform Workflows. Here's a brief overview:
- Use the Workflow Builder to create conditional branches for each stage (Employee, Manager, IT Dept, HR).
- Set conditions to show or hide pages based on the current stage.
- Configure email notifications by either having the employee add the manager's email or setting up auto-notifications with pre-configured emails.
This setup allows each role to view and edit only their respective pages and progress the form to the next stage via submission buttons.
Give it a try and let us know how it goes.

Hi Richard,
Here’s a step-by-step guide to set up your multi-stage form workflow in Jotform:
- Open your form in Form Builder and go to Settings.
- Select Workflow from the left panel and click Create Workflow to open Workflow Builder.
- Add conditional branches for each stage: Employee, Manager, IT Dept, and HR.
- Set conditions to show only the relevant page for each stage (e.g., Page 1 for Employee, Page 2 for Manager, etc.).
- Configure email notifications by adding Email elements. You can use form fields for dynamic email addresses or set pre-configured emails.
- Connect submission buttons to progress the form to the next stage in the workflow.
- For HR final review, allow access to all pages by setting no page restrictions.
This setup will guide your form through each stage with proper visibility and notifications.

Hi Richard,
Currently, we don't have a live form example to share directly. However, you can create your multi-stage workflow using Jotform Workflows by following these steps:
- Create your form with the necessary pages and fields.
- Use Workflow Builder to set conditional branches for each stage (Employee, Manager, IT Dept, HR).
- Set page visibility conditions so each role sees only their relevant page(s).
- Configure email notifications to notify the next reviewer, either via dynamic email fields or pre-configured emails.
- Connect submission buttons to move the form to the next stage.
This setup will replicate the workflow you described. You can customize and test it within your account.
