My Personal Information Was Fraudently Used

I am reaching out to report that my personal information was submitted without my consent through one of your forms.

Specifically, someone used my name and phone number to book an appointment with Veeva Chiropractices via a Jotform form. I did not submit this form and have no affiliation with the request.

Could you please investigate this incident? I would appreciate it if you could:

• Confirm the date and time of the submission,
• Provide any available information about the submitter (such as IP address, if possible),
• Assist in removing my personal data from the submission, and
• Prevent further unauthorized use of my information via your platform.

Please let me know if you need any additional details from my end. I have contacted Veeva Chiropractices and they were able to confirm that my full name and phone number were used to apply for an appointment via Jotform.

I’ve followed up with Veeva Chiropractices regarding the unauthorized appointment made using my name and phone number. They confirmed that the submission was made via Jotform on Wednesday, June 4th at approximately 1:20 PM, booking an appointment for Friday, June 6th at 10:30 AM. The appointment was reportedly scheduled by someone claiming to have injured their back at work.

The following personal details of mine were used without my permission:

• Name: Mai Dang
• Phone number: (814) 826-3976

This information was submitted fraudulently, and I am requesting that it be removed and that appropriate action be taken to prevent any future misuse.

Please let me know if you are able to provide further details about the submitter (e.g., IP address or metadata), and confirm that my data will be removed from the submission.

Since you already contacted Veeva Chiropractices already, you may ask them to remove your personal data from the submission. If the form owner violated our Terms of Use, we will suspend them. Here are some guidelines to help you avoid scams:

• Never fill out a form from an anonymous contact, an unknown organization, or a company with no website or legitimate presence.
• Never pay money to get employment
• Never provide critical information such as your SSN, banking information, or driver’s license numbers or pictures to such suspicious contacts.

I did not receive an e-mail, I found out about this Jotform submission that contained my personal information due to an appointment confirmation call from Veeva Chiropractices, they worked with me to cancel the appointment but they said I should contact Jotform about my personal information being used on their site without my permission.

How am I supposed to get my information wiped?

I just told you Veeva Chiropractices was only able to tell me the information - I don't think they're able to do anything else from what they saw for the appointment registration.

I'm literally so scared because someone signed up for a Chiropractice appointment with my information, god knows what they were going to do from there - maybe even charge that appointment to my insurance?

Since the form submission was made through a form owned by another user, in this case Veeva Chiropractices, Jotform cannot directly remove or alter submissions from forms we do not own. The form owner has control over their submissions and data. We recommend contacting Veeva Chiropractices to request removal of your personal data from their form submissions, as they have the ability to delete or purge that data on their end.

Regarding the submitter's information such as IP address or metadata, we can only access this if the form owner grants us permission to access their account and share the submitter's details. Without their cooperation, we cannot provide this information.
