Setup fom for multiple children for one family

    Hi there,

I'm working on setting up a flow that will allow registration for camp.

I'd like to be able to have parents only need to login once and then apply for 1 or more kids (1 time per form)

Then that data should go to a Jotform Table (1 entry per child)

If we are able to accept their child/ren it should send an email once per child per form.

That will include a link asking them for more info. Some of that info can come from the form and should be populated per kid.

    1) How do I let them fill out the 2nd and further forms using data from their original form and let them choose which child they are filling out for.

    2) As an additional question, is there a way to link some data at the family level and use that for the 2nd form for all children?

    Thank you

    Hi Mendy,

Thanks for reaching out to Jotform Support. I'm an AI assistant. I'll do my best to help you until someone from the support team is available.

To set up a flow where parents log in once and register multiple children with separate entries per child in Jotform Tables, you can use Jotform Tables to connect multiple forms and manage data efficiently.

    Here’s how to do it:

    1. Create a main form for parents to enter family-level data and child details.
    2. Use Jotform Tables to connect forms and manage submissions, allowing one entry per child.
    3. Set up email notifications to send individual emails per child with prefilled links for additional info.
    4. Use the Send Form Action Button in Jotform Tables to send prefilled forms for each child, linking family-level data for convenience.

This setup allows parents to fill out forms for multiple children with shared family data and receive separate emails per child.

Give it a try and let us know how it goes.

    Hi Mendy,

Thanks for reaching out to Jotform Support. I can see that you have multiple forms that are in line with your request. But to make sure we are looking into the correct one, and to guide you properly regarding your request, can you share with us the form link or URL of the main form you will use for form users to initially register?

From there, let us know which form is what you call the 2nd and further forms, so we can be in-line with the guidelines we'll cover for you. It's easy to get a form's link or URL. Let me show you how:

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
    2. In the Quick Share menu, the form URL will be shown in the middle of the page, just below Share with Link. Click on the Copy Link button and paste the copied link together with your next reply.

After we hear back from you, we’ll have a better idea of what’s going on and how to help.
