Workflow help

Hi,

I am trying to create a workflow that allows the form to be sent to one of our employees to approve and sign their name onto the PDF that the form we created populates to. I am just a little confused on how the workflow operates. I have tried to read all of the info pages but cannot figure it out. I have attached a photo of the areas that need to be filled out by an Acceleron employee before the form gets sent to the requesting physician.
