Unable to locate form

    I need help. I created a form already and paid for a Jotform account but now I don't see my form. Do I have another account and maybe I am accessing the wrong account?

Can you send me the link to your form so I can take a look at it for you? You can also provide the following payment traction details that we can use to locate your paid account where your form is associated:

    1. Last 4 digits of the credit card:
    2. Expiration date:
    3. Amount charged:
    4. Type of card (visa, AMEX, etc):
    5. Date of transaction:

    Once we hear back from you, we'll be able to help you with this.
