I need help. I created a form already and paid for a Jotform account but now I don't see my form. Do I have another account and maybe I am accessing the wrong account?

You can also provide the following payment traction details that we can use to locate your paid account where your form is associated:
- Last 4 digits of the credit card:
- Expiration date:
- Amount charged:
- Type of card (visa, AMEX, etc):
- Date of transaction:
