Form Calculation Widget Total Score Always Showing 0

    Dear Jotform Support Team,

    I am experiencing an issue with the Form Calculation widget on my form, which is intended to calculate a total score for a "Neuro Burnout Meter" quiz. Despite following the setup instructions, the calculation widget consistently displays "0" in the form preview and in submissions, even when valid options with assigned calculation values are selected.

    Here's a breakdown of my setup and what I've tried:

    Form Structure:

    • I have 10 "Single Choice" questions on the form.
    • Each question asks the user to rate an item from 1 to 5, with specific text labels for each rating (e.g., "1 = Not at all", "5 = Constantly").
    • All 10 questions are marked as "Required".

    Calculation Values Setup (on each Single Choice Question):

    • For each of the 10 Single Choice questions, I have enabled "Calculation Values" in the "OPTIONS" tab of their properties.
    • I have assigned the corresponding numerical value to each option:
    • 1 for "1 = Not at all"
    • 2 for "2 = A little"
    • 3 for "3 = Somewhat"
    • 4 for "4 = A lot"
    • 5 for "5 = Constantly"
    • I have double-checked that these values are indeed numbers (1-5) and not text or containing extra spaces.

    Form Calculation Widget Setup:

    • I added a "Form Calculation" widget to the end of my form, labeled "Total Burnout Score".
    • In the widget's settings, I have set the formula to sum all 10 of my Single Choice questions. The formula appears as: {Question1} + {Question2} + ... + {Question10} (where Question1, Question2, etc., represent the actual field names from my form).
    • All fields within the formula editor appear in blue (Field), indicating they are recognized as numerical variables, not brown/beige (String).
    • I have also tried temporarily unhiding the "Total Burnout Score" field to observe its value in real-time during the preview.

    Troubleshooting Steps Taken So Far:

    1. Verified Calculation Values: I have meticulously checked the "Calculation Values" for multiple individual questions (beginning, middle, and end of the list) to ensure they are correctly set as numerical inputs (1-5).
    2. Re-configured Formula: I have deleted the "Form Calculation" widget and re-added it, then completely re-built the formula from scratch by adding each field individually and ensuring the + operator is used between them.
    3. Browser Cache/Incognito Mode: I have cleared my browser's cache and cookies and also tested the form in an incognito/private Browse window to rule out caching issues.
    4. Tested a Simple Form: I created a separate, simple test form with only two Single Choice questions and a Form Calculation widget to sum them. This simple test form did calculate correctly, which suggests the issue might be specific to my main "Neuro Burnout Meter" form.

    Despite these efforts, the "Total Burnout Score" consistently remains 0. I am unable to proceed with setting up conditional logic for action plans without this core calculation functioning correctly.

    Could you please investigate why the calculation widget is not summing the values from my Single Choice fields? I can provide the form URL or access to my account if needed.

    As you can see, I can get other results than zero. Can you share with us a screen recording of what’s happening so we can try to replicate it on our end? You can share the video recording on any public sharing platform like Google Drive or Dropbox, or you can simply use Loom to record the video and share the video link with us.

    Once we hear back from you, we'll be able to move forward with a solution.
