Need a Sign up form with Subscription form

2 mock up attached. Need this done the right way, there will be 2 buttons on e mail. Using constant

contact and paypal

Thanks for reaching out to Jotform Support. I see you’re working on building both a sign-up and subscription form. You also mentioned sending attachments with mockups of your forms, but this thread doesn’t support attachments.

If you can take a screenshot of your mockups and share it with us, it will help us better understand what you’re looking for and find the right solution. Let me show you how to post a screenshot in our Help Center:

1. On the Help Center page, scroll down to the Your Answer section and click on the Image icon.

2. Drag and drop your image into the Upload box, or click on it and select your file.

3. Then, click on the Add button in the bottom-right corner of the window.

4. To resize it, click on your screenshot and then click on one of the boxes in the corners, and drag inward.

5. Once you're finished, click on the Post Answer button at the bottom right of the Your Answer section.

That's it. Once we hear back from you, we'll be able to help you with this.

We’re not allowed to create any forms for you, as we only provide support to JotForm users and help resolve issues they experience with the platform.  If you’d like, you can reach out to JotForm Experts, they can assist in building the form for you. Jotform is an online form builder that provides a number of tools and 10,000+ customized templates. Dragging and dropping features or elements makes it really easy to create forms. I'll help you do it:

1. On your My Forms page, click on Create Form on the top-left side of the page.
2. Choose Start From Scratch, and select Classic Form to create an empty form, or Use Template to choose among our 10,000+ ready-to-go templates (Start From Scratch was used in this example).
3. Now, in Form Builder, click on the Add Form Element menu on the left side of the page to see the complete list of elements and widgets.
4. Under the Basic, Payments, or Widgets tabs, scroll down the list, and then drag and drop the fields onto your form. And that’s it.
