Hello, I am having issues creating my form. On the form builder, it looks good, but when I preview it, many parts are missing and parts that are supposed to be hidden are showing and there is just a lot of issues I am encountering.

I have done and double checked those options and even using ChatGPT to help me, but I am still encountering issues.
