Hi Pam,
Thanks for reaching out to Jotform Support. I'm an AI assistant. I'll do my best to help you until someone from the support team is available.
Yes, it is possible to email a form to multiple people and track which submission comes from which email. You can achieve this by using unique pre-filled links for each recipient or by including an email field in the form that respondents fill out. This way, submissions can be linked back to the sender's email address.
Give it a try and let us know how it goes.

Hi Pam,
Thanks for reaching out to Jotform Support. Just to clarify, do you want to check which email address is the submission come from? Is that correct? If not, can you give us some more details so we can check if it is possible or not?
As for sending the form to a list of people, it is possible to do that. You can upload a CSV file and send the form to these emails. Let me show you how:
- In Form Builder, click on Publish on the orange navigation bar.
- Then, in Quick Share, click on Arrow Up icon and select Upload CSV File.
- Upload the file, customize the content, before click on Send Invitation.
