I have imported both the pdf and my docx to the Smart Form import to create my form. It did not work correctly (see attached screen shot)

Thanks for reaching out to us about the issue with Smart Form Import where check boxes are not showing up correctly after importing your PDF and DOCX files.
When you import a PDF or DOCX to create a Smart PDF Form, the system tries to fetch and clone the form fields from your document, but sometimes it may not detect all fields perfectly, including check boxes. This can cause the check boxes not to appear as expected in the Jotform form.
There are a few ways you can address this:
- Manually add missing fields: You can add any missing check boxes or other fields manually in the Form Builder. When you add a new field to the online form, a corresponding field will automatically be added to the PDF side, and you can map them accordingly.
- Map multiple choice fields: For single choice or multiple choice fields, you can map each selection box to individual check boxes in your PDF. This ensures the selections correspond correctly.
- Edit your original PDF externally if needed: If your PDF form needs adjustments (like adding check boxes), edit it using an external PDF editor such as Adobe Acrobat, then re-import the updated PDF to Jotform Smart PDF Forms.
- Adjust field connections: After import, you can drag and resize the fields on the PDF preview to align them properly with your original document.
Keep in mind that some field types cannot be mapped, but check boxes (as part of multiple choice fields) should be supported with manual mapping.
If you continue to experience issues, try re-uploading the PDF after making sure all check boxes are properly included in the original document.
