Thanks for reaching out to Jotform Support. You can't add input fields to the app. What you can do is create a form where you can add input fields, and add this form to the app. Let me show you how to add a form to the app.

1. In App Builder, click on Add Element + on the right side.
2. Under Basic, select Form.
3. Select the form and click on Add Form.
