We have a bounce house that we bring out seasonally. And we need to have our customer sign waiver forms electronically.

Hi Thomas,
Thanks for reaching out to Jotform Support. We offer ready-made waiver form templates that you can use right away. You can check them out using this link.
For collecting electronic signatures, we recommend using Jotform Sign, which provides a secure and seamless way for your customers to sign legally binding waiver forms. If you prefer a more traditional form setup, you can also use the standard e-signature field in our Form Builder. To get started with Jotform Sign, you can follow this guide.
Give it a try and let us know if you need any help.

Hi Thomas,
We don’t currently offer live phone support or scheduled Zoom meetings. If you have any questions, just reply to this thread, or you can create a Help Center thread from the Contact Us page. We’ll be happy to help.
Let us know if there’s anything else we can help you with.

Once we give you the form.
Can you walk me through the process?
Can the form be put on a QR code?
So that our customers can scan it on their cell phone.
Or does it have to be on a tablet?
Where will that form be stored?
How do we get access to retrieve form?
We will probably do about 100 waiver wires a month maximum.
Can you tell me what the cost would be for that?
Thx again

Hi Thomas,
We can assist you with setting up your form. You can check our plan limit and pricing on our pricing page. It is possible to share the form using QR code. You would have to embed the QR code to your website. Let me show you how:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- In the menu on the left side of the page, click on Quick Share.
- Then click on the QR code icon. You can then copy the HTML code and paste to your website.
Let us know if there’s anything else we can help you with.
