    Do you have a form that would help me keep track of business donors, their contact information and to track recurring check ins such as last time they were contact and who they were contacted by? Thank you!

    Hi CCRestore,

    Thanks for reaching out to Jotform Support. Unfortunately, we don't have a template for this requirement. But with the same form where they fill out their information along with their donations, you can filter their submissions from the Jotform Table. There you can see how many times they fill out the form, their contact information, and the donated amount. Here's how to filter submissions:

    1. On the Jotform Table, place your cursor into the Search section at the upper left.

    2. Type in the name that you want to display, and it will show the list immediately.

    3. To create a filter, click on the Filter button, select the fields and the value, and then click on the Apply Filter button.
