Redirect user to another form based on the answer

On my Pre-screening form, I need to create workflow that if they choose the "New" option under Patient Status (Single Choice Properties), they need to be redirected to another form named, New Patient Registration form. If they choose the "Current:" option and one of of the Health Package option, they need to be redirected to that Health Package form. I need help urgently please.

Thanks for reaching out to Jotform Support. To clarify, do you want the person filling out the form get redirected to a specific form, when they choose either New or Current? Can you confirm if you want to send the form to them via email, or get them automatically redirected after filling out the first form? If you want them redirect automatically, then you can create a Change Thank you page condition. Let me walk you through it:

1. In Form Builder, in the orange navigation bar at the top, click on Settings.

2. In the menu on the left side of the page, click on Conditions.

3. Now, click on the +Add Condition, and choose Change Thank You Page.

4. Finally, set up the condition like below, and click on Save.
