How can have all labels display on top of answer box?

Hello, for my Rosa T. Beard reference form, some of the labels are displaying to the left of the answer box. I selected the" top"option in the field properties paine for each question and checked save as default but it still doesn't show on top for some labels. How can i fix this issue? Thank for your help.

Thanks for reaching out to Jotform Support. I have checked your form, and it shows that it's only the Input Table has an issue. Let me show you how to fix it:

1. Open your form in Form Builder, scroll down, and select the Input Table.
2. Click on the Gear icon on the right side of the field to open Input Table Properties.
3. Under General in the Input Table Properties, uncheck the checkbox for Label Alignment.
