Thanks for reaching out to Jotform Support. The template you shared with us is for the Sales Prospecting Management Form. It is using the Section Collapse and the Heading element. Let me show you how you can create a form using these elements:

1. Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on Add Form Element. 
2. Scroll all the way down, and choose Section Collapse, then scroll up, and select Heading.
3. Click on the Section Collapse element, select the Black gear icon to open Properties.
4. Enter the details in the Section Heading, Section Heading button should be HIDDEN.
5. Choose VISIBLE for the Button Visibility.
6. Click on the Heading element, select the Black gear icon to open properties.
7. Under the GENERAL, enter the text in the Heading Text, and Subheading-Text.
