Columns on table not showing up on form, what do I do ?

Hi Andy,
Thanks for reaching out to Jotform Support. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
You can try to reset the Column view on your table and see if it will display the submitted information. It is very easy to do. Let me show you how:
1. In Tables, select Columns in the upper right part of the page.
2. Select the option Reset columns to default from the dropdown.
3. Click the Reset to Default button and you're done.
