we cant access the pca form ? we need assistance

Hi JasLinksHealthcare,
Thanks for reaching out to Jotform Support. Are you referring to the Personal Care Assistant (PCA) Competency Test? If so, it seems that your form is currently disabled, which is why you are unable to access it. Enabling the form again is easy let me show you how:
- Open your form in Form Builder and click on the Settings tab.
- Go to Form Status, and make sure that the status is Enabled.
