Can you provide me with Jotform's W9?

Good morning, I am interested in upgrading our account to the $34/month package, but I do need some information from you in order to do so as we operate out of a high school. Could you please provide me with your company's W9 so I can have our Account's Payable department add you to our system? We would also need to negotiate payment terms to be a net 30 payable by check. Is this possible?

I would also only be looking to get 1 month of the subscription to finish out the form collection we already have going. Is this an option?

If you wish to pay via Purchase Order (invoice, check), you can forward your request to quotes@jotform.com. Our team will get back to you regarding your request as soon as possible. You can also tell them your preferred payment schedule, which is on a monthly payment. However, here's a copy of our W-9 form with Jotform's details.
