Issue With Adding Options For Photo ID To The Form

I have this file - https://form.jotform.com/231775058883064 this is from prior to my arrival at this company. however i would lilke to improve the file by adding options for photo ID and forms. This form seems to be from a Jotform template so the regular way of adding pics/forms does not seem to be working.

Thanks for reaching out to Jotform Support. I cloned and tested your form, and it seems to be working properly, as I was able to add a File Upload element for uploading a photo. Check out the screencast below:

If this is not exactly what you are trying to do, could you describe what happens when you try to add the photo ID and forms? Do you see any error messages, or are the options simply not available? This will help pinpoint the issue and provide a solution to it. Additionally, if you can take a screenshot of what’s happening, that'll help us to understand better what’s going on. Let me show you how to post a screenshot to our Help Center:

1. On the Help Center page, scroll down to the Your Answer section and click on the Image icon.

2. Drag and drop your image into the Upload box, or click on it and select your file.

3. Then, click on the Add button in the bottom-right corner of the window.

4. To resize it, click on your screenshot and then click on one of the boxes in the corners, and drag it inward.

5. Once you're finished, click on the Post Answer button at the bottom right of the Your Answer section.
