Survey Responses Coming Back Blank

    I put out a survey (Training Critique) to our members and almost all of the responses coming back are blank. The few exceptions are a couple where the email address was entered (it was an optional field). I sent myself a few tests and they have also been blank,

    It was created based on a clone of a previous form that worked fine. That form was the Ask the Trainer form (https://form.jotform.com/250257606652154)

    What can I do to fix this?

    Hi Sebasticooknavhda,

    Thanks for reaching out to Jotform Support. I cloned your form and tested it to see if I could replicate the issue, and I ran into the same issue. When I looked at your form, I noticed that the correct Unique Name wasn’t entered in the Email Content settings. Take a look at the screenshot below: Survey Responses Coming Back Blank Image 1 Screenshot 70 Survey Responses Coming Back Blank Image 2 Screenshot 81 Some of the other fields might also be missing from the notification email for the same reason. To fix this, we recommend deleting the Notification Email and then adding it back. This usually resolves the issue, as it will automatically pull in the correct Unique Names for each field. Let me show you how:

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.

    2. In the menu on the left side of the page, click on Emails.

    3. On the right side of the Notification field, click on the Trash Can icon, and delete the existing Notification Email.

    Now that we've removed the old Notification Email, we'll just add a new one. Let's do it: 

    1. While you're still under the Emails tab on the left side of the page in Form Builder, click on Add Email.

    2. Next, select Notification Email.

    3. Click on the Recipients tab and enter your email address in the Recipient Email field.

    4. Now, just click on Save and you're done.

    Thank you, Cyrus! That worked perfect! I guess that's something I'll have to keep in mind when I clone forms.
