How to add a checkbox and text input using the same field?

Thanks for reaching out to Jotform Support. Just to make sure that I understood what you're asking, you want your form fillers to fill in by ticking the options applicable to them and also have the option to add their answer when the option is not listed in the choices, right? If that's what you're trying to do, you can use a Multiple Choice field for that. It's really easy to do. Let me show you how:

1. While in Form Builder, click on the Add Element menu on the left side of the page.
2. Under the Basic tab, scroll down the list and choose the Multiple Choice field.
3. Now, just drag and drop it to your form.
4. Click the Gear icon and go to Options tab to add your choices and enable the Display Other Option.
