How to send digital certificates to attendees after survey completion?

I have an upcoming conference where we have traditionally provided printed certificates for RACE CE credits at the end of the day, following the completion of a paper survey. I would love to transition away from this format and move everything to a digital system, with surveys and certificates organized by lecture.

Upon completing the survey, attendees would receive a link to their pre-made CE certificate. Is this possible through Jotform?

Thanks for reaching out to Jotform Support. If your certificate is a PDF file, then you can integrate it using Smart PDF Forms which matches their entries to the PDF. They can fill up the form and after they submit it, they will receive their certificate with their name on it via the Autoresponder Email. Let me walk you through it:

1. On your My Forms page, click on Create Form on the top-left side of the page. 

2. Choose Import Form and then Import PDF Form.

3. Now, upload your PDF file.

4. Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.

Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields. Check out the screencast below to see how to do that:

Now, to include the certificate in the email they receive, you have to attach it. Let me show you how:

1. In Smart PDF Forms, in the blue navigation bar at the top of the screen, click on Settings.
2. On the left panel, click on Emails, hover over the Autoresponder Email, and click on the Pencil icon.
3. Under the Advanced tab, enable the Attach PDF option and select the Original PDF.
4. Click on Save when you're done, and that's it.
