Same issue as MANY others have in the comments below from over a year ago. Why has this not been resolved yet?
I have done all the proper steps and no PDF attachment comes through with the TEST emails. PLEASE FIX THIS ISSUE.

Thanks for reaching out to Jotform Support. I noticed on your form the PDF document was not attached. Check out my screenshot here:
I cloned your form here in order to test whether or not I received the PDF attachment with a test email after attaching the PDF document. When I tested my cloned form, the same thing happened, so I created a ticket about it and escalated this to our Developers. Check out my screencast here:
Although we don't know exactly when it'll be fixed, we’ll circle back to this thread when we have an update.
