I would like to convert this risk questionnaire into a Jotform survey. Is it possible to convert a PDF document into a survey.
I attached a screenshot of page 1. There are two pages to the PDF document.

Hi Rich,
Thanks for reaching out to Jotform Support. Jotform makes it easy to integrate your PDF files using Smart PDF Forms. Let me walk you through it:
1. On your My Forms page, click on Create Form on the top-left side of the page.
2. Choose Import Form and then Import PDF Form.
3. Now, upload your PDF file.
4. Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.
Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields. Check out the screencast below to see how to do that:

I tried using this feature and its terrible. Doesn't come even remotely close to duplicating the original PDF. I would have to rebuild this entire form from scratch.
Form is titled Envestnet Risk Questionairre.

Hi Galzarano,
After uploading your Original PDF Document, you can manually adjust the fields in your Smart PDF Forms. Use the Basic elements within the Smart PDF Form Builder to distinguish between questions and answers effectively. Let me show you how:
- In Smart PDF Forms, click on the +Add Element button at the left side of the panel to open the Form Elements menu.
- Under the Basic tab, select the Single Choice element and add it to your form.
- Click on the Gear icon, add the Question Title to the Field Label input box.
- Then, go to the Options tab and add the Options in the Input table.
