    Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named [Imported Form] Student and Parent Sign In that’s breaking our Terms of Use by collecting login credentials.

    Suspended: Login credentials collection Image 1 Screenshot 20I've deleted the offending fields and reactivated your account. You should be able to log in now.
