    Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named Tap Money Community Matic that was tagged as phishing. We understand that this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use.

    Hi Anatolii, 

    For us to reactivate your account, we’ll need you to do the following:

    1. Send us your Business Permit Registration
    2. Send us a link to your website or proof of affiliation with a legitimate institution.
    3. Any documents that show that your are operating a legitimate service.

    Send these things above to reviewers@jotform.com with Thread# 18654641as the subject, and we’ll let you know the status of your account after we’ve done a thorough review of them.
