I need a form and share it

i need a form that i can use to collect swim suit sizes as a link that can be published

Thanks for reaching out to Jotform Support. You can check out our templates here and pick the ones that you would want to use. Once you're done, you can then share it with others. Here's how to do it:

1. In Form Builder, in the orange navigation bar at the top, click on Publish.

2. Click on Copy Link and use this to share with your recipients.
