    If you can share a link to your web page, we will gladly take a look in order to provide proper suggestion. 

    In general, it could be possible that embed code is in conflict with plugins installed on a page. I can recommend to try embedding form using iframe code: Getting-the-Form-iFrame-Code

    If you are using a WordPress page builder add the code to the raw HTML element, works fine.
