Now, coming back to your question, I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. The submission was sent to Google Sheet. Check out the screencast below to see my results:

I tried sending submissions to your Google sheet, but it shows that there are no missing submissions. Can you review your Google sheet, and see if it has 62 submissions? If you are missing submissions, you can reintegrate the form with Google Sheets integration. Removing the integration and adding it again is easy. Let me show you how:

1. In Form Builder, click on the Settings tab from the orange navigation bar.

2. Click the Integrations on the left menu and then click on Google Sheets.

3. Now, click on the Three Dots, then click on the Delete Action button from the dropdown, and then click on the Yes, Discard button on the confirmation window.

4. Then, click on the Google Sheets on the Integrations page again, select the option to create a new spreadsheet or use an existing spreadsheet, and click on the Next button.

5. Select your Google account for authentication.

6. Add the spreadsheet, and worksheet name, and select the form fields to send to the spreadsheet.

7. Finally, click on the Save button.
