Now, coming back to your question, you can create a Question and Answer form. You can use one of our 10,000+ customized templates. Let me show you how:
On the left side of the page, click on Create Form.
Click on the Add Form Element menu on the left side of the page.
Under the Basic tab, scroll down the list, choose the options that you want, and then drag and drop them onto your form.
Another way if you have PDF already, your can integrate your PDF files using Smart PDF Forms. Let me walk you through it:
1. On your My Forms page, click on Create Form on the top-left side of the page.
2. Choose Import Form and then Import PDF Form.
3. Now, upload your PDF file.
4. Add the necessary fields and customize them using the Add Form Element menu on the left side of the page.
Sometimes, the fields that are automatically created in the PDF file will be in the wrong place. If that happens, you'll have to move and resize the fields. Check out the screencast below to see how to do that:
That's it. The final preview will look like the one below:
1. Go to the Quiz Form page, and click on the Open Quiz Form button.
2. In the window that pops up, click on Allow, select the form you want to turn into a quiz, and then click on the Continuebutton in the top-right corner of the Form Picker window.
3. Click on the Cross icon to close Form Picker, then in the Options section, click on Checkbox next to Email Results to and enter the email address and subject.
4. Then, click on Checkbox next to the Show Results to User After Submission option.
5. Now, scroll down, select the correct answers for your quiz, and click on Save Quiz at the bottom left of the page and that's it.
Now, when someone finishes your quiz and submits the form, they will see their results. Check out an example below to see how it'll look:

If you want to change the email content, check the Use Custom Email Page box and customize it as you like.
Additionally, Jotform has any type of Form Element you may need. Take a look at the complete list below and get a brief description of each Form Element in Jotform. After you create your form, and if you want to embed your form on your website, we have several options for Which Form Embed Code Should You Use. We strongly advise using Iframe code. It is very simple to embed your form on a webpage. Let me show you:
In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
In the menu on the left side of the page, click on Embed.
Now, click on the embed option you want to use, and copy the code. iFrame method was used for demonstration purposes in this case.
That's it. Embed your link on your webpage and you're done.
